How to apply for NDIS assistance

Finding a Service Provider

The first port of call if you think you may be entitled to an NDIS plan is to call the NDIS on 1800 800 110. The NDIS will help you work out if you entitled to apply for an NDIS plan.

The NDIS Provider Finder lists all NDIS registered providers. Search by location and service on Provider Finder (see link below)

There are also a number of private provider directories that can be very helpful by searching for NDIS provider services.

How to make a complaint

If you feel that you need to make a complaint about any NDIS service provider then you should do so.
You can make a complaint directly to the service provider by phone, email or in person. Download the How to Make a Complaint flyer here:

How do I choose a Plan Manager

You are free to chose any registered NDIS Plan Manager that you wish to.

Some participants decide chose a local plan manager because they know the local service providers, however this is not crucial. Plan Managers can provide an excellent service thousands of miles from your location.

If you prefer a more personal service, having someone you can talk to who remembers who you are then Able Plan Manager is for you.

How do I chose to be Plan Managed

When you have your initial planning meeting ask your NDIA planner or local area coordinator (LAC) to set up your plan as “plan managed” and include “Improved Life Choices” in your budget.

If your plan has already commenced you can contact the NDIS on 1800 800 110 and ask for a review.

Let them know that you would like to use Able Plan Manager as your NDIS registered plan manager.
